Patient Self Referral information West Hertfordshire Wellbeing Service

It can often be difficult to cope alone, whether your issues are caused by problems you face every day or by events that have happened in your past – talking therapies can help you.

The West Hertfordshire Wellbeing Service is an Improving Access to Psychological Therapy Service. This is a government initiative to provide easily accessible, evidence-based treatments, to the general population experiencing common mental health problems such as anxiety and depression.

The aim of the service is to help people across the county to be able to access psychological therapy and social care support, to help them achieve better psychological health and wellbeing, enabling them to gain or remain in employment and to participate in daily living activities

Self referral

If you are over 16 years and registered with a GP in Hertfordshire, you can self refer to the Wellbeing Service:

Alternatively, you can download the self-referral form and return it via post or email at the address on the bottom of the form
